Your Health Care Benefits

The Inlandboatmen's Union of the Pacific National Health Benefit Trust is managed by a Board of Trustees comprised of both union and employer representatives. Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful links, and frequently asked questions regarding your benefits provided by the Plan. Medical and Prescription Benefits

The Active Employee Plan makes the Trust Medical Plan available to you as one option for your medical, and Rx coverage.  The Trust PPO Plan utilizes the Premera Blue Cross Network for Medical and Moda Health for Prescription benefits. To get the most out of your benefits, it is best to use providers that participate in the networks. To locate a participating provider in the Premera Network or for questions regarding medical benefits and claims, please contact (800) 713-5373. Dental Benefits


The Active Employee Plan makes the Trust Dental Plan available to you as an option for dental coverage. Covered dental services are paid at 80% of Reasonable and Customary Charges for preventative and basic services, and 50% of Reasonable and Customary Charges for other dental services. There is no Deductible. There is a calendar year maximum benefit payment of $2,000 per Covered Individual. Vision Benefits


The Vision plan is provided by the Trust to accompany the Trust Medical plan. Covered vision services are paid at 80% of Reasonable and Customary Charges. There is no Deductible. There is a calendar year maximum benefit payment of $300 per Covered Individual. For questions regarding your vision benefits, please contact the Trustt Office at (503) 224-0048.

Life Insurance

The Trust provides life insurance to employees only, regardless of medical plan enrollment.  Dependents are not eligible for this benefit.  The Plan provides $15,000 of life insurance coverage through Standard. Your life insurance becomes effective on the date you qualify for group health and welfare benefits.

 

 Time Loss

The Trust provides Time Loss Benefits to employees only, regardless of medical plan enrollment. Dependents are not eligible for this benefit. This benefit is designed to partially replace a participating employee’s income while disabled.
