Q.View hours or contributions my employer paid on my behalf?
A.To view hours or contributions paid on your behalf, you’ll need to log into your account. Once you are logged in, go to My Benefits and select Health Care Benefits. At the bottom of the screen, you will be able to view your Health Care Contributions. If you have questions regarding the hours or contributions paid on your behalf, please contact NNEBT at 800-258-9732.
Q.See if I am currently eligible for benefits?
A. To see if you are currently eligible for benefits, you’ll need to log into your account. Once you are logged in, go to My Benefits and select Health Care Benefits. Under the primary member information, you will see a box labeled, My Health Care Eligibility. If you have questions regarding your eligibility, please contact NNEBT at 800-258-9732.
Q.See which dependents are covered under my plan?
A.To see which dependents are covered under your plan, you’ll need to log into your account. Once you are logged in, go to My Benefits and select My Info. On the right side of the screen, you will see a box labeled, Dependent(s). If you have questions regarding your dependents, please contact NNEBT at 800-258-9732.
Q.Inquire about the status of a submitted dental, health club, massage or hearing claim?
A. To inquire about the status of a claim, you’ll need to log into your account. Once you are logged in, go to My Benefits and select Health Care Claims. You will see five tabs that are labeled accordingly: Health Club, Massage, Hearing, Other, and Dental. Select the claim you are inquiring about, and then you will see your claims and your dependents’ claims. If you have questions regarding your claims, please contact NNEBT at 800-258-9732.
Q.Add or remove a dependent from my plan?
A. To add or remove a dependent from your plan, you will need to complete a Vital Information Form and submit supporting documentation. No dependents can be added or removed without the appropriate documentation and, if applicable, the existence of a valid qualifying event. To access the Vital Information Form, first, you’ll need to log into your account. Once you are logged in, the Vital Information Form can be found by selecting My Benefits and then My Info. Select the link provided to view the form. After the form is completed, please mail or fax your form to the NNEBT.
