Medical Benefits

Your medical benefit network is provided by Premera Blue Cross. To get the most out of your benefits, it is best to use a provider that participates in the Premera network. Prescription Benefits

Your prescription benefits are provided by MedImpact. To get the most out of your benefits, it is best to use a pharmacy that participates in the MedImpact network. Please visit their website at the link at the right side of this page for network providers and further information.

Dental Benefits

Your Dental Benefits are provided by Moda Health.  To locate a participating provider or for questions regarding your dental benefits and claims, please visit their website at the link at the right side of this page.

Vision Benefits

Your Vision Benefit Network is provided by VSP.  Using a participating provider that accepts the VSP network will ensure that you get the most out of your vision benefits provided by the Plan. Life and Accidental Death & Dismemberment (AD&D) Insurance

Active Life Insurance benefit is $20,000 and AD&D Insurance benefit is $20,000.  Please contact the Trust Administrative Office for more information.

Accident & Sickness Weekly Indemnity Benefits

This is a weekly benefit up to $350.  Please contact the Trust Administrative Office for more information.
