NOTICE OF RIGHT TO REQUEST TAX NOTIFICATION FORM 1095-B
The Plan has elected not to send IRS Form 1095-B (“Health
Coverage”) for the 2022 tax year, as permitted by IRS Regulations. You do not need to file the form with your
federal tax return, but some members may need the form to comply with state
reporting requirements. Members and
beneficiaries may request for a copy of their Form 1095-B by:

INFORMATION REGARDING CORONAVIRUS - RELATED DISTRIBUTIONS
The CARES Act permitted participants under age 59 ½ to take coronavirus-related distributions from their retirement accounts without incurring the 10% penalty for early distributions. The income from the distribution could also be taxed over several years. The distribution had to be “made on or after January 1, 2020, and before December 30, 2020.”
In late December 2020, the Consolidated Appropriations Act of 2021 and the COVID-Related Tax Relief Act of 2020 amended the CARES Act but did not extend the cutoff date. We will be denying applications received in 2020 that could not be processed by 12/30/2020 and all applications received on or after 12/30/2020.
