Medical, Dental, Vision and Prescription Benefits

For “Active Members under age 65” your medical, dental and prescription benefit network is provided by Excellus BlueCross BlueShield. To get the most out of your benefits, it is best to use a provider that participates in the Excellus BlueCross BlueShield network. To locate a participating provider or for questions regarding benefits and claims, please contact (877) 253-4797. For "Retired Members over 65 or Medicare Primary Members" your medical is provided by United Healthcare.  For assistance with this please contact Labor First 585-207-0208 or toll free 844-780-6363.

Your prescription benefit is provided by United Healthcare.  If you utilize the mail order with OptumRx Home Delivery, you will receive a 90 day supply for the cost of a 30 day supply.

Your dental coverage is provided by Excellus BlueCross BlueShield.

Other Benefits

The Fund self-insures other benefits such as Short Term Disability, Death Benefit, Accidental Death and Dismemberment, for members that are eligible and active in the Local 86 Insurance Plan. These benefits have special requirements and conditions for you to be eligible for them. Please see your current Summary Plan Description or call the Fund Office at (585) 235-1515 for more details.

UR Medicine Employee Assistance Program

UR Medicine EAP is a benefit to all employees and their family. UR Medicine EAP will provide professional guidance when personal or work-related problems have become difficult to manage alone. UR Medicine EAP provides confidential and immediate help with a wide range of concerns including: workplace stress, marital and relationship issues, family problems, parent/child difficulties, drug and alcohol abuse, depression and anxiety, financial pressure, and many other issues.
