Your Health Care Benefits

The Board of Trustees is pleased to welcome you to the new website for the Operating Engineers Local 234 Health and Welfare Fund. Within this website, you will have access 24 hours a day, 7 days a week to commonly requested forms, useful links, and frequently asked questions regarding your benefits provided by the Plan. Medical and Prescription Benefits

Active Employees, Non-Medicare Retirees, and Medicare Retirees are allowed to enroll in the Medical and Prescription Plans offered by the Fund.  Active Employees and Non-Medicare Retirees may enroll in the Alliance Select Medical Plan through Wellmark of Iowa. To get the most out of your benefits, it is best to use a provider that participates in the Alliance Select network. To locate a participating provider or for questions regarding benefits and claims, please contact (800) 524-9242. Your prescription coverage will be under the Alliance Select/BlueRx Complete Plan also through Wellmark of Iowa. They can be reached at (800) 524-9242. Retirees of Medicare age (65 or older) may enroll in the Local 234 Medicare Employer PPO Plan through Wellmark for medical and prescription coverage.   For questions regarding benefits and claims, please contact the Local 234 Health and Welfare Fund office.  

Dental Benefits


Your dental benefit network is provided by Blue Dental through Wellmark of Iowa. Using a participating provider that accepts the Blue Dental network will ensure that you get the most out of your dental benefits provided by the Plan. Vision Benefits

Your vision benefit is provided as a reimbursement plan. You must pay for services upfront and provide proof of services via an itemized receipt before reimbursement can be made. Reimbursements will be up to $400.00 every 24 months.  All member and spouse eligibility starts from 1/1/2020 through 12/31/2021 and will reset 1/1/2022 and every 2 years thereafter.  Dependent children through the age of 26 are eligible for $150.00 every 12 months each calendar year.  Dependent eligibility also starts from 1/1/2020 through 12/31/2020 and each calendar year thereafter.  For questions regarding your vision benefits and claims, please contact Local 234 Health and Welfare Fund office at (844) 568-6335 or (515)207-3875, extension 5600 or 5601.

Hearing Aid Benefits 

Effective 1/1/2020, Members only are eligible for a $1,000 benefit every five (5) years for hearing aids when medically necessary and prescribed by an audiologist or other doctor specializing in ear, nose, and throat (ENT).  This benefit is a reimbursement plan.  Member must provide proof of services via an itemized receipt before reimbursement can be made.   

Short Term Disability Benefits

Weekly disability benefits are available to members that are actively working (not on lay-off) for a maximum of 13 weeks for a non-work related injury or illness and a maximum of 4 weeks for a work related injury or illness.  A claim form must be completed by your physician.  Contact your Health & Welfare office for further information.   

Life/ AD&D Insurance

As a Retired or Active Employee, the Death Benefit shown below will be paid to your beneficiary after proof of death is received by the Fund Office. Active Employees’ Spouse and child(ren) are also covered as shown below:

Retired Employee = $2,000
Employee = $7.500
Spouse = $1,000
Child older than 14 days = $500

As an Active Employee, you are also covered under the accidental death and dismemberment (AD&D) benefit.
