When do I become eligible for benefits?

At age 59 1/2 or upon termination of employment.

When can I start Participating in the Plan?

A.

There is no waiting period. A member can start participating in the Plan on day one of employment with a Contributing Employer.

Q.

What is a Plan Year?

A.

January 1 - December 31

Q.

Do I need to do anything to enroll in the Plan?

A.

Yes, you must complete Union and Enrollment forms.

Q.

When do I become vested?

A.

100% upon Elective Deferral.

Q.

How do I become eligible to withdraw money from my account?

A.

At age 59 1/2, termination of employment, or a qualifying hardship event.

May I borrow against my 401K Plan?

A.

Yes

Q.

How is my account invested?

A.

Self-directed, a member chooses the investments.

Q.

What happens to my account when I die?

A.

Member's benefits are paid to the Beneficiary on file.

How do I designate a Beneficiary for my Individual Account?

A.

By completing the Designation of Beneficiary form at the time of enrollment.

Q.

What if I get divorced?

A.

This would be stipulated by your Divorce Decree and Marital Settlement.

Q.

How do I apply for a benefit?

A.

You will log on to the John Hancock website and begin filling out the loan application. John Hancock will then mail you a physical application. Once this step is complete you will need to download the certification of marital status form, this form needs to be notarized. We also will need a copy of your photo ID. Once you have all of these documents completed you can mail them in or e-mail them to: megan.sherman@benesys.com

How do I apply for a withdrawal?

A.

You can request the withdrawal application from John Hancock (833-388-6466). Once this step is complete you will need to download the certification of marital status form, this form needs to be notarized. We also will need a copy of your photo ID. Once you have all of these documents completed you can mail them in or e-mail them to: megan.sherman@benesys.com
