2025 Annual Dependent Audit
The 2025 Dependent Verification process is required by the Board of Trustees of the Employee Painters' Trust for Participants who have dependents. Carefully review the information presented and make the necessary changes to ensure that the information is accurate as well as verify that only Eligible Dependents are covered. This process must be completed by April 4, 2025, otherwise claims will not be paid. You do not need to complete this process if you do not have any dependents, but if you need to add an Eligible Dependent or have an address change, you can do that at this time.

New Dental Provider - Delta Dental
Effective
January 1, 2025, your dental benefit network is provided by Delta Dental. Using
a participating provider within the Delta Dental network will ensure that you
get the most out of your dental benefits provided by the Plan. To locate a
participating provider or for questions regarding your dental benefits and
claims, you may call Delta Dental’s customer service team at (800) 554-1907 or text at (833) 604-1246 Monday – Friday between 6 am – 5 pm PST or visit us at www.DeltaDentalWA.com
NOTICE OF RIGHT TO REQUEST TAX NOTIFICATION FORM 1095-B
Form 1095-B provides you with information about your healthcare coverage, including who was covered, and when the coverage was in effect. You may not need to file a Form 1095 with your federal tax return, but some members may need the form to comply with state reporting requirements. If members and beneficiaries have not already received an IRS Form 1095-B via U.S. mail, they may request that the Plan send another copy of their Form 1095-B to them via U.S. mail. You may make this request by email, phone, or written request. The Plan will mail you the form within 30 days of receiving your request. Please include your name and address in your request. Your request MUST include: (1) your Plan’s name, (2) the member’s name, (3) your name if you are not the primary member, (4) the address you would like the form sent to and (5) the phone number we can call if we have any questions. Elixir is Now MedImpact
Earlier this year, Elixir was acquired by MedImpact, the nation’s largest independent pharmacy benefit and health solutions company. There is no impact to your pharmacy benefits and the Rx BIN and Rx PCN will remain the same, meaning there is no action for you at this time. In December, new ID cards will be mailed out for your pharmacy benefits along with the Delta Dental information.
Employee Assistance Program (EAP) Services
The
Trustees of the Employee Painters’ Trust (EPT) are pleased to announce that
Canopy Wellbeing has been selected to provide Employee Assistance Program (EAP)
services.
This new benefit is now available to eligible Members and
Dependents up to 26 years of age. The EAP provides free, confidential, and
professional assistance, including assessment, problem solving, and referral
for a wide variety of personal or family concerns. Services include four (4)
confidential face-to-face counseling sessions, legal consultations, simple
wills, financial coaching, child and eldercare resource retrieval services,
identity theft services, and more.

Maternity Leave Benefit
The Board of Trustees are pleased
to provide a new benefit under the Plan – Paid Maternity Leave Program!
Beginning April 1, 2022, the Plan will offer eligible Participants with a Paid
Maternity Leave Benefit. This new benefit program provides benefit program
provides benefits for pregnancy (pre-delivery/birth) for a maximum of six (6)
months and post-delivery/birth for a maximum benefit of six (6) weeks or eight
