Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful links, and frequently asked questions regarding your benefits provided by the Plan. Starting with the 2025 tax year report issued in January 2026, the Ohio Bricklayers Health and Welfare Fund will no longer automatically mail Form 1095 to participants. If you would like a copy of your Form 1095, please contact the Fund Office at (833) 289-4921 for a copy to be mailed to your address on file. A copy will be mailed to you at no cost within 30 days of your request. Please note, as of this tax year, that the Fund does not email the Form 1095.

Medical and Prescription Benefits

Your medical and prescription benefit network is provided by Anthem Blue Cross and Blue Shield and Sav-Rx. To get the most out of your benefits, it is best to use a provider that participates in the Anthem Blue Cross and Blue Shield network and in the Sav-Rx network.
