Frequently Asked Health Care Questions


Q. Whom should I call if I have questions about my eligibility?
A. The Benefit Office at (314) 656-1072.
Q. How do I add my new baby or spouse?
A. You must get a Vital Information Form from the Benefit Office. Fill it out and return it to the Benefit Office no later than 30 days from the birth of your baby or date of marriage. A Marriage Certificate is required, and a Birth Certificate is required.
Q. Will my child who is age 19 to age 26 be covered under the Plan?
A. Dependent children can remain on the Plan until the end of the month in which they attain age 26, see the Summary Plan Description for more detail on what qualifies a child as a valid dependent under the Plan.
Q. How can I be sure my claim will be paid?
A. There are certain expenses that your Plan does not cover. If you are unsure about a particular expense, contact the Benefit Office. 
Q. What if my spouse also has medical coverage through his/her employer?
A. We suggest that you contact the Benefit Office, and we will assist you in determining who the primary payer is. If you are secondary for your dependents, always file the claim with the primary payer, first.
