You must submit a marriage license to the Benefit Office within 30 days of your marriage or for a new baby you must submit a birth certificate to the Benefit Office within 30 days of the birth of your child. Send the document to:

For all other dependent coverage changes, please contact the Benefit Office.

Your dependent is covered until he/she reaches the age of 26 provided that he/she does not have other group health coverage. You will need to complete the Dependent Child Age 19-26 form available from the Fund Office. Please submit to:
