Who is eligible to become a Participant in the Plan?

A.

Once you have worked the initial 800 hours you become eligible for Insurance.

Q.

What if I don’t work enough hours to gain eligibility for the month?

A.

If you didn't work enough hours per month then you will receive a billing. Once you receive the bill which will have an HRA form.  Any further questions please call the Benefits Office.

Q.

How do I maintain my monthly Health Care coverage?

A.

To maintain the health care coverage and not receive a bill you would have to work 110 hours per month.

Q.

How do I make a payment towards the continuation of my Health Care coverage?

A.

Whom should I call if I have questions about my Health Care eligibility?

Who are my eligible dependents?

A.

Eligible dependents are: Spouse and legal children 26 and under

Will my child(ren) who is/are age 19 through age 26 be covered under the Plan?

A.

Yes children are covered through age 26 under the medical plan.

Q.

How do I add my new baby or spouse to my insurance plan?

A.

You should contact the Benefits Office of any changes within 30 days. They will need a copy of the Birth Certificate and/or marriage license.

Whom should I contact if I'm getting a divorce and what documents do I need to submit?

A.

Call the Benefits Office within 30 days and send in a copy of the divorce papers.

How do I obtain a replacement Medical and Prescription Card?

How is the HRA reimbursement money deposited into my account?

A.

For every hour you work, money is put into your HRA account based on the contract you are under.

Is there a minimum HRA reimbursement amount?

A.

Yes, minimum of $20.00

Q.

Can I use the HRA reimbursement account for Self payments? What about COBRA?

A.

Yes, you can use the HRA for self-payment.

Q.

For HRA reimbursement, is there a filing limit?

A.

You are only limited by your HRA balance. You can only be paid out what you have in your account.

Can I lose my HRA balance?

A.

The only way you will lose your balance is if you are not eligible for twenty-four (24) consecutive months.
