Your Medical Benefits 'In Brief'

The Board of Trustees is pleased to welcome you to the Health Care website for the Teamsters Local 631 Security Fund for Southern Nevada. Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful highlighted links, and frequently asked questions regarding your benefit information. Medical Plans

There are two Medical plans available to the Teamsters Local 631 Security Fund for Southern Nevada members. The two options are the Exclusive Provider Organization (EPO) Plan, and the Indemnity Plan.


Plan maximums, copays, deductibles

The plan maximums, copays, and deductibles for the EPO are determined by the Trust.

The PPO Indemnity Plan offers coverage with 20% coinsurance and/or applicable copays, and an out of pocket maximum. If using PPO providers, there will be no calendar year deductible.

The Non-PPO indemnity plan has a 50% coinsurance with no out of pocket maximum and a $500 calendar year deductible.


Hospital, medical, surgical

Coverage is provided for approved hospital, medical, and surgical visits with the EPO plan and the Indemnity Plan. For information regarding either plan, please call (877) 304-6702.


Prescriptions
