Medical Benefits

Your medical benefit network is provided by Medical Mutual of Ohio. To get the most out of your benefits, it is best to use a provider that participates in the Medical Mutual of Ohio network. To locate a participating provider, please call (800) 382-5729.

Medicare Advantage

Your Medicare Advantage plan is provided by Humana.  You have the freedom to visit any provider that accepts Medicare and agrees to bill Humana. Humana Medicare Customer Care: (800) 733-9064, Monday - Friday, 7 a.m. - 8 p.m. Central Time.

Vision Benefits
