Your medical benefit network is provided by CareFirst Blue Cross Blue Shield. To get the most out of your benefits, it is best to use a provider that participates in the CareFirst Blue Cross Blue Shield network. Prescription Benefits

Your prescription benefit network is provided by CVS Caremark. To get the most out of your benefits, it is best to use a provider that participates in the CVS Caremark network. Dental & Vision Benefits

Dental and vision benefits are provided by the Metropolitan DC Paving Industry Employees Health & Welfare Fund.

For vision, you can go to the eye doctor of your choice and you will be reimbursed for your expense for a complete eye exam by a qualified optometrist or ophthalmologist and a pair of lenses and frame, up to the maximum annual benefit for adults. For covered individuals under age 19 (pediatric vision benefits) there is no annual maximum for essential benefits.

For dental, you may see the Dentist of your choice; however, the Fund will offer the Cigna Dental Shared Administration DPPO Plus Network beginning October 1, 2019. Benefits are subject to the Fund’s annual, maximum and essential health benefits provisions.
