Medical Benefits Your medical benefit network is provided by Anthem. To get the most out of your benefits, it is best to use a provider that participates in the Anthem network. To locate a participating provider or for questions regarding benefits and claims, please contact Anthem at (833) 610-1799. Life Insurance
As an employee, you are offered basic life/accidental death and dismemberment (AD&D) and supplemental life insurance for yourself. You can only purchase basic life insurance with AD&D for your spouse, and basic life insurance for your dependent child(ren) if you elect basic life insurance for yourself.
