    File an Application for Withdrawal. Applications can be downloaded from this website or requested from the Trust Fund Office.  

     

    How do I designate a beneficiary? 

    Complete a Beneficiary Designation form and file it with the plan. The Beneficiary Designation Form can be downloaded from this website or requested from the Trust Fund Office.
