Your Health Care Benefits

The Board of Trustees is pleased to welcome you to the new website for the San Francisco Culinary, Bartenders & Service Employees Welfare Fund. Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful links, and frequently asked questions regarding your benefits provided by the Plan. Medical and Prescription Benefits

Your medical and prescription benefit network is based on your enrollment elections. To enroll in the plan, complete the Benefit Enrollment Forms available on the Healthcare documents/forms page and return to the Benefit Office.

To get the most out of your benefits, it is best to use a provider that participates in the network you have elected. To locate a participating provider or for questions regarding benefits and claims, please contact your Medical/Rx Provider at the phone number listed below or click on the link on the right side of the page to connect to their website.

Medical/Rx Plan Options for Active Employees or Non-Medicare Retirees:

Dental Benefits

Upon meeting eligibility requirements, your dental benefit network is based on your enrollment elections. To enroll in the plan, complete the Benefit Enrollment Forms available on the Healthcare documents/forms page and return to the Benefit Office.

Vision Benefits

Your vision benefit network is provided by VSP. Using a participating provider that accepts the VSP network will ensure that you get the most out of your vision benefits provided by the Plan. AIDS Benefits

The AIDS Benefit is a reimbursement plan for certain qualifying expenses for Active Employees and Retirees who received this benefit as an active employee who meet the eligibility criteria. Contact the Benefit Office at (844) 492-9157 if you have any additional questions regarding the AIDS Benefit.

Child and Elder Care Benefits

The Child and Elder Care Benefits Program provides reimbursement for a portion of the cost of covered expenses for a Participants’ Eligible family members and is available to Active Employees whose Employer contributes to this benefit. Refer to the Child and Elder Care Benefits Program Supplemental Summary Plan Description for additional information. Supplemental Sick Leave and Supplemental Vacation Benefits

Supplemental Sick Leave and Vacation Plans pay certain benefits that are generally based on long-term employment and applicable only to Active Employees whose Employer contributes to these benefits. Whether a participant is eligible for these benefits is entirely dependent on the terms of the collective bargaining agreement between the Union and the participant’s employer.
