Monthly Benefit Statement Updates: Dear Sheet Metal Worker Local 33 Participant: The Sheet Metal Workers Local 33 Benefit Fund Office recently mailed your new revised monthly Benefit Statement. The new look still has all of your important information regarding your hours, contributions, eligibility and health enrollment elections. Please note, under the Health and Welfare only members under the Cleveland Health Plan will have eligibility and enrollment information. If you are a participant in Tri-County Building Trades Health Fund you will receive a separate statement and would need to contact that Fund Office for Health Eligibility status. Also, please look for important messages that may be printed monthly on the back of your statement. If you have any questions, do not hesitate to call us at: (216) 267-3344 or Toll Free (888) 424-7488. Sincerely, The Sheet Metal Workers Local 33 Fringe Benefit Funds Office
