Medical Benefits
Your medical benefit network is provided by Blue Cross Blue Shield of MA, to locate a participating primary care physician in the HMO Blue New England network, please call (800) 821-1388 or click on the BCBSMA link on the right side of the page to connect to their website. PLEASE note you must select a PCP to receive care otherwise you may be denied or have to pay for services out of pocket.
Prescription Drugs
Your prescription drug benefit is provided by BCBSMA. To get the most out of your benefits, it is best to use a pharmacy that participates in the BCBSMA network. To locate a participating pharmacy, please click on the BCBSMA link on the right side of the page to connect to their website. PLEASE discontinue using the Allegiant Rx ID card immediately and use your new BCBSMA id card for prescription benefits.
Vision BenefitsYour vision benefit network is Davis Vision. Using a provider that participates with Davis Vision will ensure that you get the most out of your vision benefits provided by the Plan. Dental Benefits Your dental benefit network is provided by Blue Cross Blue Shield of MA Dental Blue Freedom. Using a participating provider that is contracted with the BCBSMA Dental Blue Freedom network will ensure that you get the most out of your dental benefits provided by the Plan. Life Insurance
As a member, you are offered basic life/accidental death and dismemberment (AD&D) insurance coverage. The information pertaining to these benefits can be found on this website under the Health Care / Documents tab.
Employee Assistance Program
All eligible members and dependents are covered by the Employee Assistance Program which provides confidential, professional counseling with alcohol dependency and drug abuse. To receive confidential assistance, please contact Modern Assistance Programs, Inc. at (617) 849-2004.
