Frequently Asked Pension Questions


Q. I’m going through a divorce, what does the Plan require?
A. The Plan requires a Qualified Domestic Relations Order (QDRO) to be filed with the courts should an ex-spouse be entitled to pension benefits. Your attorney may obtain details and assistance by writing to the Fund’s Office. If you have any questions, please Contact the Fund’s Office at 216-520-1644.
Q. How can I have my address changed?
A. All address changes must be submitted in writing. Complete the Address Verification Change Form located on the website for your convenience. Mail all address changes to Bricklayers Local No. 5 Ohio Funds, 6200 Rockside Woods Blvd. N., Suite 210, Independence, OH 44131
Q. If I want an estimate on my retirement, who do I call?
A. You can request an estimate from the Fund’s Office at 216-520-1644. An estimate will be mailed to you within 5-7 business days. If you have been divorced in the past a finalized divorce decree and settlement agreement is required prior to an estimate being mailed to you.
Q. Who do I notify if my spouse has passed away?
A. The Fund’s Office needs to be notified when a member or their spouse passes away. Please contact the Fund’s Office at 216-520-1644. A copy of the death certificate will need to be provided to the Fund’s Office.
Q. How can I have my direct deposit changed?
A. A Direct Deposit Authorization Form must be completed and returned to the Fund’s Office for your benefit to be direct deposited into your account. The form is located on website for your convenience.
Q. How do I go about changing my federal and state tax withholding from my pension retirement check?
A. You can have tax withholding changed at any time during the year by completing a new federal and state tax withholding form and mail it to the Fund’s Office. The federal and state tax withholding forms are located on the website for your convenience.
