You may be eligible for an additional 13 weeks if (1) you must stay in the hospital or (2) your physician fills out the proper forms to say that after 13 weeks, you are totally disabled. For this purpose, totally disabled means you cannot perform the duties of your job for at least 12 more months because of your physical condition. |
