You become a Participant when you have worked one Hour
of Service for a contributing Employer. You will then have an Accrued Benefit
Account in your name.

There are three ways you may collect your benefits:
- Normal Retirement – When you reach Normal retirement
age of 52 and cease working.
- Separation Benefit
– When you have reached 24 consecutive months with no Employer contributions, and
completely ceased all work in the trade, whether self-employed or work for an employer
not in the Union, you may be eligible for a Separation Benefit.

Disability Benefits – When you are totally and permanently disabled from any further
employment in the work jurisdiction of the Union.
If you qualify for one of the above reasons, please contact the Fund Office for
the appropriate application forms.(Link available on website.) You will need to
provide certain documentation in addition to the application, such as photo identification
and birth certificate copies for yourself and your spouse, your marriage license,
any and all documentation from previous marriages, such as divorce decrees, Qualified
Domestic Relations Orders (QDRO’s), or death certificates.Both you and your spouse
will be required to complete the application, and certain portions require a Notary
Public to witness your signatures.

It may take several weeks. Once your completed application
is received in the Fund Office, we will consult with the Union Hall as to your eligibility.Applications
must be reviewed by the Board of Trustees at their regularly scheduled meetings.Meetings
are not scheduled every month.Your application must be received at least ten days
prior to the meeting, or else it will be held until the next scheduled meeting.

If you are legally married when you die, your spouse is
your legal beneficiary. If you are not legally married, you may complete a Beneficiary
Form naming one or more beneficiaries. (If you are legally married, but wish to
designate someone other than your spouse, your spouse will need to sign a consent
form. Please contact the Fund Office for the appropriate form.) If you are not married
and have not completed a Beneficiary Form, benefits would be paid to your children
if you have them, or if not, to your parents if living, or to your siblings, or
to your estate, in that order.

No. There are no provisions for loans or hardship distributions.

The Trustees have selected a variety of investment options
for you. You may view your account via the Internet and decide where you would like
to place your investments. If you do not make any selections, your account balance
will be placed in a default investment based on your age and targeted retirement
date.

When contributions are first received on your behalf,
a letter will be sent to you containing your Relius/Sungard user name and password.
The website is www.glassworkers357dc.org. Enter the website address and your user
name and password carefully.

Many system-generated user names and passwords are highly
secure. It may be difficult to distinguish the number “1” from the lower case letter
“l”. Also the upper case letter “O” may be mistaken for the number zero. If your
password contains these characters, try different combinations.

After three incorrect attempts, the system locks out a
user in order to protect your account security. Your account will be reset by the
end of the following business day and you may try again.
