Medical
and Prescription Benefits
The Board of Trustees has made three medical plans available to you and your
dependents. You and your dependents can have medical benefits provided by
Providence, Kaiser Permanente, or the Trust Plan described in the Trust Benefit
Booklet. To locate a participating provider or for questions regarding your

Dental Benefits
You also have
three dental plans available to you and your dependents provided by Kaiser
Permanente, Willamette Dental Group, or by the Trust Plan. To locate

Vision Benefits
Your vision benefit network is provided by VSP vision benefits, unless you have
elected Kaiser Permanente for your medical benefit, in which case Kaiser Permanente
also provides vision benefits. Using a participating provider that accepts the
**Vision Plan** network will ensure that you get the most out of your vision
benefits provided by the Plan. To locate a participating provider or for

Regardless of
the medical and dental plans you choose, you and your dependents are eligible
for the employee assistance benefits described in the Benefit Booklet. You are
also eligible for time-loss benefits, life insurance benefits, and accidental
death and dismemberment benefits.
