Medical Benefits
Your medical benefit network is provided by Anthem Blue Cross. To get the most out of your benefits, it is best to use a provider that participates in the Anthem Blue Cross network. Prescription Benefits
Your prescription benefit network is provided by Optum Rx. To get the most out of your benefits, it is best to use a provider that participates in the Optum Rx network. To locate a participating provider or for questions regarding benefits and claims, please contact

Dental Benefits
Your dental benefit network is provided by Cigna. Using a participating provider that accepts the Cigna network will ensure that you get the most out of your dental benefits provided by the Plan. To locate a participating provider please contact Cigna at

Vision Benefits

Telehealth Benefits

Your telehealth benefits is provided by LiveHealth Online. LiveHealth allows you to have a virtual visit with a board-certified doctor using your smartphone, tablet, or computer with webcam. Doctors are available 24/7, 365 days a year. When you need to see a doctor, simply go to livehealthonline.com or use the LiveHealth online mobile app.      

Life Insurance
The Fund has contracted with Sun Life Assurance Company of Canada for the provision of life insurance coverage. As an employee, you are offered basic life/accidental death and dismemberment (AD&D) for yourself. For more information or for questions regarding benefits and claims, please contact (800) 927-4357.  Health Reimbursement Arrangement (HRA)
