Medical Benefits

Your medical benefit network is provided by Aetna. To get the most out of your benefits, it is best to use a provider that participates in the Aetna network. Prescription Benefits

Your prescription benefits are provided by CVS/Caremark. To get the most out of your benefits, it is best to use a pharmacy that participates in the CVS/Caremark network. To locate a participating provider, please call (800) 764-7287 or visit their website at the link at the right side of this page.

Dental Benefits

You and your family have the choice to visit any dentist. Delta Dental maintains a network of dentists who have agreed to bill Delta Dental directly and accept a negotiated fee (discount). Allowable charges for out-of-network dentists are paid based on what 90% of the dentists in the area would consider reasonable. If your dentist is outside the Delta Dental network you may be responsible for any additional amounts above the reasonable fee

Vision Benefits

Your vision benefit network is provided by VSP. Using a participating provider that accepts the VSP network will ensure that you get the most out of your vision benefits provided by the Plan. Medicare Health Plan
