Q.

How do I become a Participant in the Plan? 

A.

The first date that the Plan receives a contribution from you or your employer on your behalf, whichever comes first 401(k) – To Participate, Complete a tax deferred savings authorization form Contributions from 1 to 25%. You can change the deferral up to three times per year. A form needs to be completed to not participant. Catch up contributions are allowed. 

Q.

Does the Servance and Annuity/401(k) Plan affect Social Security benefits in any way? 

A.

You may consult with the Social Security Administration to determine whether your pension benefit has an impact towards you receiving a Social Security benefit. 

Q.

Can Servance and Annuity/401(k) be paid or assigned or garnered to others? 

A.

Q.

If benefits are denied, may a retiree or beneficiary appeal? 

A.

A retiree or beneficiary may appeal any denial of benefits by submitting a written letter to the Board of Trustees at P.O. Box 99039 Troy, MI 48099. For more information, please contact the Fund Office at (443) 573-3639 or Toll Free (855) 505-0462.

In addition to the tax deferred savings authorization form, what other documents do I have to submit to the Fund office? 

A.

You must submit birth certificates for you and your spouse (if applicable) along with your marriage certificate and any divorce decrees with Property Settlement Agreements (if applicable). For more information, please contact the Fund Office at (443) 573-3639 or Toll Free (855) 505-0462.

How do I request a withdrawal or hardship from my account?

A.

You must create an account with Empower at www.empower.com/participant. Once you are registered you can request withdrawal or hardship. Please contact Empower for more information at 1-855-756-4738.

I recently moved; how do I change my address? 

A.
