must complete and submit a new enrollment form to the Fund Office with a letter
from the hospital indicating the baby's date of birth and the name of each
parent. You will have 60 days to submit a copy of the baby's birth certificate.
If the birth certificate is not received the baby would be dropped from the
plan until it is received by the Fund Office.
Adding a Spouse: You must complete and submit a new enrollment form to
the Fund Office with a copy of your marriage certificate. |
