Q.

Who is eligible to become a Participant in the Plan?

A.

If you are an Employee within the jurisdiction of the Stone and Marble Masons of Metropolitan Washington, D.C. Health and Welfare Fund and employed by a participating Employer who has signed a Collective Bargaining Agreement with the Union, you will become eligible for benefits with the required hours.

Q.

What if I don’t work enough hours to gain eligibility for the month?

A.

If your benefits have been terminated you will again become eligible for benefits on the first day of the month following any period within the preceding twelve months in which you have been credited with at least 950 hours of employment, for which contributions have been made by one or more contributing Employers. Periods of proven disability up to a maximum of twelve months are not counted as periods of unemployment and you can be credited with 8 hours for every week day that you are receiving Weekly Accident and Sickness (A&S) Benefits from the Fund. If you exhaust your A&S benefits, but continue to be disabled or are receiving Workers' Compensation benefits, you will continue to be eligible up to a maximum of twelve months by being credited with 8 hours for each week day that you continue to be disabled, with proof of continued disability.

How do I maintain my monthly Health Care coverage?

A.

You and your qualified dependents will continue to be eligible for benefits from month-to-month provided you continue to be credited with no less than 950 hours of employment during the twelve consecutive months preceding the first of each month.

How do I make a payment towards the continuation of my Health Care coverage?

A.

Active members can make COBRA payments by check or money order.  Employees of a Delinquent Employer may also self-pay contributions for up to (but no more than) 3 months while the Fund pursues collection.  Self-pay will be based on actual hours worked for the Delinquent Emloyer multiplied by the contribution rate in effect at the time of the delinquency. The Employee will be reimbursed once the Fund collects the delinquent contributions from the Employer.   Additionally, unemployed participants and those who have received the maximum disabiity credit may be eligible to continue coverage through self-pay under specific conditions.  Contact the Fund Office at 410-872-9553 for more information on self-payments.

Who are my eligible dependents?

A.

Dependents are defined as your spouse and children from date of birth up to 26 years of age.

Q.

Will my child(ren) who is/are age 19 through age 26 be covered under the Plan?

A.

The term “children” shall include your children by blood or adoption and foster children that are under the age of 26.

How do I add my new baby or spouse to my insurance plan?

A.

Complete and submit a new Enrollment Form to add a spouse or child, along with your Marriage Certificate to add a spouse and/or birth certificate to add a child.

Whom should I contact if I'm getting a divorce and what documents do I need to submit?

How do I find a dentist or find out if my dentist participates with Cigna Dental?

A.

You or your dependents can obtain dental services from any licensed dental provider; however, the Fund participates in the CIGNA Dental Shared Administration DPPO Plus Network and you will save money if you go to in-network dentist. Q.

If I have Medicare, who does my provider submit the claim to?

A.

The Fund participates in a Medicare Crossover Program and your claims should be submitted electronically by your provider to Medicare and they will forward the claim information electronically to the Fund. If you wish to submit yourself, the paper claim along with your Medicare EOB should be submitted to the Fund Office at the following address: Stone and Marble Masons Health & Welfare Fund, PO Box 1302, Troy, MI  48099.

Who are my Vision benefits through and how can I contact them?

A.

Q.

How do I become eligible for Accident & Sickness Benefits?

A.

You must be an active, eligible member and unable to work due to an accident or injury that is not work related.  You must have written verification of your condition by a physician which also provides the expected length of time you will be unable to return to work. Contact the Fund Office at 410-872-9553 for further information about the Weekly Accident & Sickness benefits and to obtain a claim form.

Q.

Does the fund offer any extensions? 

A.

 The initial period of Accident & Sickness benefits provides up to 10-weeks of benefits while you are unable to work due to your non-work-related condition. This benefit can be extended for up to 10 additional weeks if you remain unable to work and this is verified by your treating physician.

Q.

How much does the Accident & Sickness Benefit pay per week? 

A.

The weekly benefit is $435 and is subject to taxes.

Q.

How long can I collect an Accident & Sickness Benefit? 

A.The initial benefit period is up to 10-weeks. An extension of up to 10 additional weeks is available if you remain unable to work and this is verified by your treating physician.
