includes member information and resources for the UA Local 190 Health Care
Plan and the Individual
HRA Plan, and more! Within this website, you now have access 24 hours a day, 7
days a week to commonly requested forms, useful links, and frequently asked questions
regarding your benefits provided by the Plans. As always, please feel free to

additional questions or concerns.

Medical and Prescription Benefits
Your medical and prescription benefit network is provided by Blue Cross Bule Shield of Michigan (BCBSM). To get the most out of your benefits, it is best to use a provider that participates in the networks. Dental Benefits
Your dental benefit network is provided by Delta Dental Michigan. Using a participating provider that accepts the Delta Dental network will ensure that you get the most out of your dental benefits provided by the Plan. Vision Benefits
Your vision benefit network is provided by Davis Vision.
