    The Board of Trustees is pleased to announce that the Health & Welfare Plan is partnering with Activate Healthcare and opening two Collective Union Heath locations for eligible Participants and their covered family members.  Collective Union Health facilities provide convenient comprehensive Primary and Acute care and wellness services, delivered by experience physicians and advance practice providers, exclusively to Local 525 members at no cost.

    The 2 locations are:

    1607 E. Windmill Lane, Suite 100, Las Vegas, NV  89123

                                      and

    911 N. Buffalo Drive, Suite 113, Las Vegas, NV  89128.

    The Windmill & Buffalo locations are NOW OPEN!  Call (702) 960-4465 (Windmill location) & (702) 960-4441 (Buffalo location) for an appointment or stop in to meet the staff.

    NEW PROVIDER NETWORK EFFECTIVE 1/1/19 

    The Plan has chosen Anthem Blue Cross as the PPO provider for the Indemnity Medical Plan and the Exclusive Provider Organization (EPO) Plan effective January 1, 2019.  Anthem Blue Cross is now the current provider network. To obtain the maximum possible benefits from the Plan, you still must use Behavioral Health Options (BHO) for behavioral health services and Navitus for prescription drug benefits.  Finally, the current Hospital network in the Las Vegas area is not changing and will apply to both the PPO and the EPO Plan.

    To find a doctor using the Anthem website:

    Go to www.anthem.com

    -Click on the Menu on the upper left side of the web page

    -Select "Find a Doctor" under the Care section in the middle of the page

    -Under "Search as a Member", go to the OR section that asks for "Identification Number or prefix (first three values) and enter IFP

    Then you can find a provider in the network by entering the type of provider, the specialty of the provider, and your location.  You can also limit the selection to those doctors who are accepting new patients.

      *QUEST LABORATORY IS NO LONGER A CONTRACTED PROVIDER*

    If you have lab work done by Quest after January 1, 2019, it will be considered Out of Network and your out of pocket expense will be higher.  You are responsible to inform your ordering physician to ensure they do not send lab requests to Quest.  If your doctor orders lab work using a Quest lab slip, you should take it to LabCorp (Anthem contracted lab) or to one of the Collective Union Health clinic locations.  Lab work that is done at the Collective Union Health clinic locations is no cost to you.

    Remember:  When working outside the Local 525 jurisdiction, travelers' hours are pro-rated and reported less timely; therefore, your healthcare coverage could be affected.   Please contact the Trust Fund office for more information and to avoid possible loss of coverage when working outside the Local 525 jurisdiction.
