The Board of Trustees is pleased to welcome you to the new Health Care website for the Utah Pipe Trades Health and Welfare Plan. Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful highlighted links, and frequently asked questions regarding your benefit information. The
Utah Pipe Trade Trust provides all of your medical, prescription drug, weekly
disability benefits, and benefits under the Employee Assistance Program (EAP).
It also provides insured benefits: life and accidental death and dismemberment
insurance. Through this website you will be able to access all of your medical
claims information as well as useful links to all your medical providers.
Medical Benefits
For active members, early
retirees and non-Medicare dependents of retirees, CIGNA will administer your medical claims and
facilitate payment to your health care providers. Starting January 1,
2020, contact Cigna at (800) 853-2713 or www.CignaAccess.com if you have questions about
your medical benefits or the status of a claim. BeneSys will continue
administering your other benefits and member/dependent eligibility
For Medicare retirees,
Medicare will continue to be primary and Cigna will be secondary, for covered
medical claims.
Prescription
Drug Coverage
The Plan covers outpatient prescription drugs through an independent pharmacy
benefit manager (PBM) drug card program. Prescription Coverage is currently
offered through Express Scripts.
Weekly
Disability Benefits
Weekly disability benefits are only available to Active Employees.
Employee
Assistance Program (EAP)
The Employee Assistance Program (EAP) is a resource designed to help you and
Covered Dependents deal with many personal or relationship issues that may be
causing distress, such as marital difficulties, family problems, stress and
anxiety, depression, grief or loss, drug/alcohol addiction, and legal issues.
The EAP is administered by Blomquist Hale Consulting.
Dental
and Vision Coverage
The
current temporary dental and vision benefit will continue through December 31,
2021. The Fund will continue reimbursing you for up to $1,250 per family
