    Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful highlighted links, and frequently asked questions regarding your benefit information. As always, please feel free to contact the Benefit Fund Office at (314) 739-2973.


     Effective January 1, 2026, CarelonRx (which is part of Anthem) will replace MedImpact as the Fund’s prescription drug benefit manager.

    You should be receiving two new I.D. cards in the next few days, which will be replacing your existing cards. 

    The first card will be your new medical and prescription card.

    The second card will be for Dental and Vision only.

    If you need assistance, please call the benefit office at 1-800-483-0101 or have your pharmacist call: 1-833-296-5039


     Mail order:  Members should be calling the Pharmacy member services number on the back of the ID card for all pharmacy related questions.  If the question is related to specialty or mail order, the Carelon RX team can ensure to you are routed to the appropriate area to assist. The Pharmacy member services number is: 1-844-483-2675.

    IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

    NOTICE OF RIGHT TO REQUEST TAX NOTIFICATION FORM 1095-B

    In accordance with IRS regulations , this notice is intended to provide you with information related to obtaining a copy of your IRS Form 1095-B from the Operating Engineers Local 513 Welfare Fund.   Form 1095-B provides you with information about your healthcare coverage, including who was covered, and when the coverage was in effect. 

    The Plan has elected not to send IRS Form 1095-B (“Health Coverage”) for the 2025 tax year, as permitted by IRS Regulations.  You do not need to file the form with your federal tax return, but some members may need the form to comply with state reporting requirements.  Members and beneficiaries may request for a copy of their Form 1095-B by:

    • Calling (314) 739-2973 between 7:30 AM and 4:00 PM CST
