Medical and Prescription Benefits (non-Medicare Participants)
Your medical and prescription benefit network is provided by Blue Cross and Blue Shield PPO. To get the most out of your benefits, it is best to use a provider that participates in the Blue Cross and Blue Shield PPO network. Medical and Prescription Benefits (Medicare Eligible Participants)

For Participants and Dependents who are eligible for Medicare benefits, either through retirement over the age of 65, or due to disability, your medical and prescription drug coverage is provided by Humana Advantage. For information on enrolling under Humana or if you have questions on your self-payment premium, contact the Benefits Office Eligibility Department. You may also contact Humana Medicare Advantage at (800) 733-9064 to speak with a Group Medicare Customer Care representative if you have any questions about the benefits, or access the Humana link provided to you on this webpage. Included under the Documents Tab, you can view the Humana Medicare Advantage Benefit Summary for the medical and prescription drug coverage. 

Dental Benefits

Your dental benefit network is provided by Delta Dental. Using a participating provider that accepts the Delta Dental network will ensure that you get the most out of your dental benefits provided by the Plan. Vision Benefits 

Your vision benefit network is provided by EyeMed. Using a participating provide that the EyeMed network will ensure that you get the most out of you vision benefits provided by the Plan. Prescription Benefits

Your prescription benefit network is provided by Envision RX options. To get the most out of your benefits, it is best to use a provider that participates in the Envision RX options network.
