Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful links, and frequently asked questions regarding your benefits provided by the Plan. IMPORTANT HEALTH
COVERAGE TAX DOCUMENTS
The Plan has elected not to send IRS Form 1095-B (“Health
Coverage”) for the 2025 tax year, as permitted by IRS Proposed Regulation section 1.6055-1(g). You do not need to file the form with your
federal tax return, but some members may need the form to comply with state
reporting requirements. Members and
beneficiaries may request for a copy of their Form 1095-B by:
- Calling (402) 401-2345 between 7:30 AM and 4:30 PM CST

Your request MUST include: (1) your Plan’s name, (2) the member’s name, (3) your name
