| A. | If you get a divorce (whether before or after retirement), your spouse may be entitled to receive a portion of your pension benefit in accordance with the terms of a Qualified Domestic Relations Order (QDRO). A QDRO may affect the amount of pension benefit you will receive or are receiving. A copy of QDRO procedures will be provided to you, free of charge, at your request by contacting the Fund Office Steamfitters and Plumbers Local Union 464, Pension Fund 3930 South 147th Street, Suite 100, Omaha, NE 68144, Phone (402) 401-2345, or Toll Free (855) 725-3551 or fax (402) 592-2352. You will also need to notify the Fund Office of your divorce so that your spouse is removed from your health coverage and is provided with any applicable forms to continue coverage. |
