Q.

Who is eligible to become a Participant in the Plan?

A.

Participants covered by a collective bargaining agreement. 

Q.

What if I don’t work enough hours to gain eligibility for the month?

A.

135 covered hours are required each month to continue coverage. Amounts over that will remain in your hour bank up to 675 hours.  If you do not have enough hours, you have the option to continue coverage by making timely payments to the Trust Office.  If you have questions about maintaining your coverage or how to make a payment, please call us at 800-811-8851.

Q.

When do I first become eligible for coverage?

A.

You will become eligible for coverage beginning with the first day of the second calendar month following an accumulation of 540 hours in your Hour Bank.

How do I make a payment towards the continuation of my Health Care coverage?

A.

Who are my eligible dependents?

A.

Legal Spouse, Natural children, stepchildren, adopted children and children placed with you for adoption, foster children, children for whom you have legal custody who reside with you, the member, in a regular parent-child relationship. From birth to Age 26

Q.

Will my child(ren) who is/are age 19 through age 26 be covered under the Plan?

A.

Yes, dependent children are eligible to remain covered until age 26, regardless of student status. 

Q.

How do I add my new baby or spouse to my insurance plan?

A.

You must submit legal documentation such as a birth certificate or marriage certificate to the Trust Fund Office, along with a completed Enrollment Form. You can download the Enrollment Form off of this website located under "Forms" and mail it to the Trust Fund Office or log in and upload the form and certificates under the “Documents to Submit” button.

Whom should I contact if I'm getting a divorce and what documents do I need to submit?

A.

Please call the Trust Fund Office and advise that you are in the process of getting a divorce or have already gotten divorced. You will also need to submit a FULL copy of your Dissolution of Marriage Judgment, QDRO (Qualified Domestic Relations Order) and child custody order assigned by the court.

You can find a doctor by visiting http://www.aetna.com and clicking on "Find a Doctor." Then go to the bottom right and select "Search our Public Directory" or you can call 800-811-8851.
