    NOTICE OF RIGHT TO REQUEST TAX NOTIFICATION FORM 1095-B

    You do not need to file a 1095 form with your federal tax return, but some members may need the form to comply with state reporting requirements. Members and beneficiaries may request a copy of their Form 1095-B by:

    Your request MUST include: (1) your Plan’s name, (2) the member’s name, (3) your name if you are not the primary member, (4) the address you would like the form sent to and (5) the phone number we can call if we have any questions.

    Participants should sign up for the new patient portal at members.eversidehealth.com.  You can also download the Everside app from the App Store or Google Play.  You may be familiar with Activate, the company who manages the Union Family Health Center offices.  Please note, Everside Health is the parent company of Activate, so you may see the Everside Health name popping up when accessing information and/or services related to the Union Family Health Center.  The name change will not affect the services; Activate will remain the entity that manages the health centers.

    Full primary care services with no out-of-pocket expenses from your NEW Union Family Health Center (powered by Activate Healthcare).

    Your Union Family Health Center will provide access to no-cost medical care and prescriptions for members and their families covered by the Health & Welfare Plan.

    1)  No-cost primary care - No out of pocket cost for visits with an experience on-site health care provider to guide and support your health care initiatives. 

    2)  No-cost prescriptions (on select formulary) - No out of pocket cost for approximately 50 high-quality generic drugs when prescribed by the on-site clinician.

    3)  No-cost lab work - High quality commonly ordered labs at no cost to you at the Union Family Health Center.

     Know Your Plan

     

    Most Plan Participants have an option of two Medical and Dental Plans. Each Plan has specific rules that you must follow in order to receive the best benefits. It is important that you understand the rules of the Plan that you are covered under (see Preferred Plan termination below).  Members working as a Helper or CE/CW are only eligible for coverage under the Flex Plan.

     

    To see what Plan you are currently covered under, you can refer to the front page of your monthly benefit statement or log in above and click on Member Benefits. 

     

    Here are a few IMPORTANT facts that you should know about your plan in order to receive the best benefit available (detailed information regarding each plan can be found under the Documents tab once you've logged into this website): 


    Union Family Health Center Plan (UFHC) - YOU MUST OBTAIN ALL PRIMARY CARE SERVICES AT ONE OF THE UNION FAMILY HEALTH CENTER LOCATIONS LISTED BELOW, or claims will be denied!  You must receive specialist referrals from the UFHC primary care provider or specialist claims will be denied!  You should use Anthem providers when obtaining all medical services or benefits will be reduced.  Primary care services for anyone over 3 years old must be rendered at the UFHC!

       

    PPO Plan - You should use an Anthem provider for all medical care or benefits will be reduced and you will pay more out of your pocket.  If you choose to use a non-Anthem provider, you will be responsible for higher out of pocket costs.

     

    Diversified Dental (Indemnity Dental plan) - By using a Diversified Dental dentist, you will obtain discounted services and will not be responsible for charges over and above the contracted rate.  This allows your benefits to go further since the Plan's payments for each service is less.


    Nevada Dental Benefits (NDB - DHMO plan) - Under the DHMO plan, you will know your set cost for most dental services, which is typically a small copayment.  This plan has no annual maximum; however, you must select a Nevada Dental Benefits provider in advance of receiving services.

    Medicare Retiree Plan Change 

    Effective January 1, 2022, the Medicare medical and prescription drug plan that was in place prior to 2021 was reinstated as an option for you to enroll. Please contact the benefit office at (702) 415-2188 with any questions.
