Q. Whom should I call if I have questions about my benefits or eligibility?

Q. How do I add my new child or spouse? 
A. Contact the Benefit Office for the necessary forms and submit them with the required documentation without delay. A Marriage Certificate and Birth Certificates are required to add dependents.
Q. Will my child who is age 19 to age 25 be covered under the Plan?
A. Only if he/she is attending college full-time and the monthly self-contribution is paid on a timely basis. Verification of full-time college enrollment is required each semester. Contact the Benefit Office for the necessary form to enroll your dependent for Class O coverage.
Q. Does my plan cover conditions related to an auto accident?
A.

No.

Q. How can I be sure my claim will be paid?
A. There are certain expenses that your Plan does not cover. If you are unsure about a particular expense, contact the Benefit Office.
Q. What if my spouse also has medical coverage through his/her employer?
A.We suggest that you contact the Benefit Office, for assistance in determining who the primary payer is. If you are secondary for your dependents, always file the claim with the primary payer first. 
Q. Who do I have my prescription coverage through?
A.Your prescription coverage is through Envision RX Options.
Q. How do I find out if my Dentist participates in the Dentemax Dental Network?
A.You can call Dentemax toll free (800) 752-1547 or visit their website at www.dentemax.com. 
Q. If I become disabled, how do I apply for weekly short term disability benefits?
A. Contact the Benefit Office to determine your eligibility for weekly disability benefits and for the necessary forms to be completed by you, your Attending Physician and your Employer.
