Your Healthcare Benefits


The Board of Trustees is pleased to welcome you to the Roofers Local 30 Combined Benefit Funds' website. Within this website, you will now have access 24 hours a day, 7 days a week to commonly requested forms, useful highlighted links, and frequently asked questions regarding your benefit information. Medical and Prescription Benefits
Your medical benefit network may be provided by Independence Blue Cross (IBC) and prescription through Future Scripts.  To get the most out of your benefits, it is best to use a provider that participates in the network. To locate a participating provider or for questions regarding benefits and claims, please contact Independence Blue Cross (IBC) or Future Scripts. Dental Benefits
Your dental benefit network may be provided by United Concordia. Using a participating provider that accepts the United Concordia network will ensure that you get the most out of your dental benefits provided by the Plan.  To locate a participating provider or for questions regarding your dental benefits and claims, please contact United Concordia. Vision Benefits
Your vision benefit network may be provided by Vision Benefits of America. Using a participating provider that accepts the Vision Benefits of America network will ensure that you get the most out of your vision benefits provided by the Plan.  To locate a participating provider or for questions regarding your vision benefits and claims, please contact Vision Benefits of America. Life Insurance Benefits
 
The Fund has contracted with Ullico Life Insurance Company for the provision of life insurance and AD&D coverage. As an employee, you are offered basic life/accidental death and dismemberment (AD&D) for yourself. Dependent and Supplemental Insurances are not provided by this policy. To file a claim, please contact the Benefit Office at (877) 280-8874.
