Your Health Care Benefits
The Board of Trustees is pleased to welcome you to the new Health Care benefits website for the Teamsters and Food Employers Security Trust Fund. The medical plan options (including prescription drugs), along with the Dental Plan, Vision Plan and AD&D benefits are listed below.
A Benefit Program is applicable to you and your Dependents only if it is provided for under the terms of the Collective Bargaining Agreement between your Employer and your Union which is currently in effect, under which you are employed, and for which your Employer specifically contributes.
If you are a retired Employee, you and your Dependents will be eligible for the Hospital-Medical benefit and Prescription Drug programs if: (1) you meet the eligibility requirements and, (2) these benefits are provided under the terms of the current Collective Bargaining Agreement between your last Employer and your Union which covers the position in which you were formerly employed, and for which your last Employer specifically contributes to the Trust Fund.
The Teamsters and Food Employers Security Trust Fund provides five Benefit Programs for Active Employees and their Dependents as well as two Benefit Programs for Retired Employees and their Dependents.
The Teamsters and Food Employers Security Trust Fund offers
the following plans: Blue Cross PPO, Aetna HMO, and Kaiser HMO.
The Teamsters and Food Employers Security Trust Fund
maintains this plan to help cover many of the costs of your family’s
prescription drug needs. All eligible
Active Employees and Retired Participants enrolled in the Indemnity Medical and
HMO programs (except for members of Kaiser Senior Advantage) are eligible for
prescription drug benefits provided through a service contract with Broadreach
Medical Resources, Inc (BMR). If you are
a member of Kaiser’s Medicare Advantage Plan (Senior Advantage), you will
receive your prescription drug benefits directly from Kaiser.
The Teamsters and Food Employers Security Trust Fund offers
you a choice of dental care programs - a PREPAID DENTAL PLAN which requires you
to use one of a list of "Dental Offices" which are part of the Plan,
or an INDEMNITY DENTAL PLAN which permits you to go to any Dentist of your
choice. In addition, for Indemnity Dental Plan Participants, the Trustees have
contracted with Anthem Blue Cross to provide services at
special rates when you obtain services from a panel Dentist. You are urged to review and compare the Dental
plans carefully so that your choice of a plan will enable you to best maintain
your dental health and that of your Dependents.
The Vision Care Program is administered by the Board of
Trustees of the Teamsters and Food Employers Security Trust Fund, through a
service agreement with Vision Service Plan.
Vision Service Plan (VSP) is a joint venture of various state vision
service corporations. Through these
service corporations and their panel of over 14,000 providers, professional
vision care is provided to Participants enrolled in VSP by the Trust Fund. Care
may be obtained from a VSP panel provider or any other provider subject to the
conditions of plan benefits.
Death and Accidental Death and Dismemberment benefits are
provided only for active Employees. The
Trust Fund does not provide Death and Accidental Death & Dismemberment
benefits for your Dependents.
Podiatry Benefits
The Plan has entered into an exclusive service
agreement with a “Closed Panel” of Podiatrists called the Podiatry Plan
Organization of California (PPOC). Podiatrists
specialize in disease, injury and surgery of the feet. The Plan pays 100% of covered services
provided by a Podiatry Plan Organization Podiatrist. If you or your Dependents wish to utilize the
services of a Podiatrist, you MUST use a Provider who is a member of the
Podiatry Plan Organization.
