Death and Accidental Death
And Dismemberment Benefits - For Active Employees
Death and Accidental Death and Dismemberment benefits are provided only for active
Employees. The Trust Fund does not provide Death and Accidental Death & Dismemberment
benefits for your Dependents.
DEATH BENEFITS - $10,000
The proceeds of an Employee’s Death Benefit will be paid (in the absence of a specific
designation of beneficiary as provided below) in the following order to the living:
1. legal spouse or Registered Domestic Partner; if none, then
2. children; including legally adopted children, if none, then
3. parents; if none, then
4. brothers and sisters; if none, then
5. executor or administrator (does not include Public Administrator)
Two or more persons entitled to benefits will share equally unless otherwise designated
by you. If you would like to designate a beneficiary other than the one dictated
by the above order of preference, you may do so by obtaining and completing a beneficiary
designation form from the Trust Fund Administrative Office.
The designation must be made upon the form approved by the Board of Trustees and
the completed form must also be notarized. The designation of beneficiary shall
be effective only upon the Trust’s receipt of a properly completed form. The beneficiary
may be changed at any time by filing a new beneficiary designation form with the
Trust Fund Office.
If the beneficiary is a minor (either by preference or designation) the Employee
may complete a custodial designation form and designate a custodian to receive the
benefits on behalf of the minor. The form must be filed with the Trust Fund Office.
If a custodial designation is not filed and a minor is the beneficiary, the death
benefit shall be payable to the natural parent of the child if the benefit is $10,000
or less. If the benefit exceeds $10,000, a custodian must be appointed by the Superior
Court.
DISMEMBERMENT BENEFITS
In the event that you sustain any of the following losses caused solely by an accident, on or off the job, the indicated benefit will be in addition to any other benefits payable under the Plan.
Loss of two limbs, sight of both eyes, or loss of one limb and sight of one eye…………$10,000
Loss of one limb or sight of one eye………………………………………………………………………………$5,000
