Plans offered to participants under the Teamsters and Food Employers Security Trust

Prescription Benefits For Active Employees And Dependents
The Teamsters and Food Employers Security Trust Fund maintains this plan to help
cover many of the costs of your family’s prescription drug needs. All eligible Active
Employees and Retired Participants enrolled in the Indemnity Medical and HMO programs
(except for members of Kaiser Senior Advantage) are eligible for
prescription drug benefits provided through a service contract with Broadreach Medical
Resources, Inc (BMR). If you are a member of Kaiser’s Medicare Advantage Plan (Senior
Advantage), you will receive your prescription drug benefits directly from Kaiser.
Through a service contract with Broadreach Medical Resources, Inc. (BMR) and the
RESTAT Pharmacy Network, the Trust Fund pays benefits for covered prescriptions
provided your name is included in the list of eligibles for Prescription Program
coverage at the time you obtain a covered prescription. The BMR/RESTAT participating
pharmacy will bill the Trust Fund and will receive payment for the prescription
directly from the Trust Fund.
You may either go to a BMR/RESTAT participating pharmacy to fill your prescriptions
(Walk-in Program) or fill your prescription through the mail (Mail Order Program).
A list of participating pharmacies can be found on page 42 of your Summary Plan
Description. Please Note: Wal-Mart and Walgreens are no longer participating
pharmacies.
Prescription Benefits For Retired Employees And Eligible
Dependents
Unless you are enrolled in Kaiser’s Senior Advantage Program, refer to Prescription
Drug Program for Active Employees and Retired Participants enrolled in the Indemnity
Medical Plan or Aetna HMO.
The following information, benefits, exclusions and limitations describing the Prescription
Program apply only to pharmacy benefits available to retirees enrolled in Kaiser’s
Senior Advantage program.
If you are enrolled in Kaiser's Senior Advantage Program, you may purchase prescription
drugs through a Kaiser pharmacy for a $10.00 co-payment for each prescription or
refill, up to a 100-day supply.
If you are enrolled in Kaiser's Senior Advantage Program, you may purchase maintenance
medications through the Kaiser mail order program for a $10.00 co-payment per prescription
for a 100-day supply. There is no annual maximum. However, you must go through the
Kaiser pharmacy in order to receive benefits.
