Medical and Prescription Benefits

Your medical benefit network is provided by CareFirst (BCBS) and your prescription benefit network is CVS Caremark.  To get the most out of your benefits, it is best to use providers that participate in the networks. Dental Benefits

Your dental benefit network is provided by Fidelio Dental. To get the most out of your benefits, it is best to use a provider that participates in the Fidelio Dental network. Vision Benefits

Your vision benefit network is provided by NVA. To get the most out of your benefits, it is best to use a provider that participates in the NVA network. Disability Benefits

Life/AD&D Benefits

In the event of your death while covered, the benefit office, upon receipt of a death certificate, will submit a claim to MetLife for payment according to the Schedule of Benefits under your Plan.
