Death and Accidental Death And Dismemberment Benefits
- For Active Employees
The Plan provides Death and Accidental Death and Dismemberment coverage for Employees
only. The Fund does not provide this coverage for your Dependents.
Death Benefit.................................................................$15,000
Beneficiary - If you die while you are eligible as an active employee,
the Death Benefit will be paid to your beneficiary or beneficiaries last designated
by you before your death. If your designated beneficiary or beneficiaries is/are
not living at the time of your death or if no beneficiary or beneficiaries has/have
been named, payment will be made in the following order of preference to your living:
- Spouse or Registered Domestic Partner
- Children, including legally adopted children
- Parents
- Brothers and sisters
- Executor or administrator (does not include Public Administrator)
Two or more persons entitled to benefits will be paid equal shares unless otherwise
designated by you. If you would like to designate a beneficiary or beneficiaries
other than the one dictated by the above order of preference, you may do so by obtaining
and completing a beneficiary designation form from the Fund Administrative Office.
The designation must be made upon the form approved by the Board of Trustees and
the completed form must also be notarized. The designation of beneficiary or beneficiaries
shall be effective only upon the Fund Administrative Office’s receipt of a properly
completed form. The beneficiary or beneficiaries may be changed at any time by filing
a new beneficiary designation form with the Fund Administrative Office. If the beneficiary
or beneficiaries is/are a minor or minors (either by preference or designation),
the Employee may complete a custodial designation form and designate a custodian
to receive the benefits on behalf of the minor. The form must be filed with the
Fund Administrative Office. Custodial designation may be changed at any time by
filing a new custodial designation form with the Fund Administrative Office.
