Vision Care Plan For Active Employees And Dependents
The Vision Care Program is administered through a service agreement with Vision
Service Plan (VSP). VSP is a joint venture of various state vision service corporations.
Through VSP and its panel of over 14,000 providers VSP Panel Providers, professional
vision care is provided to Participants and Dependents.
You or your Dependents can receive services from a vision care provider who is not
a VSP Panel Provider. If you choose to do so, you must send the provider’s itemized
statement of charges along with the VSP claim form to VSP for reimbursement to you
for allowable charges in accordance with the Reimbursement Schedule.
If you go to a Non-Panel Provider there is no assurance that the schedule will be
sufficient to pay for the examination or the glasses.
