Q. As a Kaiser member, can I get my eye exam and materials with Kaiser?
A. No, as a Kaiser member, you may not get your eye exam in a Kaiser facility. 
Q. Will my child(ren) who is/are age 18 through age 26 be covered under the Plan? 
A. Yes, coverage is extended to your child(ren) until their 26th birthday.

Pension

Q. How do I go about changing my federal and state tax withholding from my pension retirement check?
A. You can have tax withholding changed at any time during the year by completing a new federal and state tax withholding form and mail it to the pension office. A form is available on this website. 
Q. How can I have my direct deposit changed? 
A. Whenever you have a change in your bank account a new direct deposit Authorization form must be completed. A form is on the Website for your convenience. 
Q. If I want an estimate on my retirement, who do I call?
A. You must call the pension office at 925-208-9994 and an estimate will be calculated and mailed to you. 
Q. How can I have my address changed?
A.

You can simply complete the Address Verification Change Form located on the website and mail it to the Pension office at P.O. Box 1917 San Ramon, CA 94583.

Supplemental Pension

Q. I'm no longer working and I would like to take a distribution from my Supplemental Pension Plan.
A. Your account balance must be under $10,000.00 and out of the Sheet Metal Industry for 18 months. A Supplemental Application must be completed and mailed to the Pension Office at P.O. Box 1917 San Ramon, CA 94583.
Q. What type of distribution options do I have, if I want to take a distribution?
A. You have the option of a Partial Lump Sum once a year, Lump Sum, Transfer Funds into an IRA, 50% Joint and Survivor Annuity, Single Life Annuity, Specified Monthly Payments, Specified Period payment, 100% Joint and Survivor Annuity.
Q. Can I take a loan against my Supplemental Pension Plan?
A. Yes, you must contact NWPS at 1-844-447-1170 (ph), 1-408-298-1180 (fax), or by email at 104loans@kandg.com.

Vacation

Q. Could my union dues be deducted from my vacation account? 
A. Yes, you can pick up a form to authorize the deductions from the dispatch office, or the credit union.
Q. Could my vacation money be deposited into my own personal financial institution? 
A. No.
Q. How do I open an account with the Credit Union to receive my vacation pay?
A. You would need to contact the credit union at the numbers listed above and complete the necessary paperwork.

SUB

Q. What should I do if I apply late with EDD?
A. If you wait and apply with EDD a few weeks after your last day worked, you will need to submit a letter of appeal when you submit your first payment stubs. Your appeal will be reviewed and approved or denied based on the circumstances.
Q. When will I receive my SUB payment? 
A.

For those check stubs received by 5:00 PM on Wednesday, money will be deposited into your vacation account at the Local 104 Credit Union on Thursday by 12:00 PM.

NO CHECKS ARE ISSUED BY THE TRUST FUND 

Q. When should I apply for unemployment with EDD?
A. Immediately! In order to receive SUB benefits you must be receiving State Unemployment Insurance (EDD).
