| A. | - First fill out a SUB application located at your local dispatch office.
- Forward to your local business agent to sign off on it (WE MUST HAVE A BUSINESS AGENT SIGNATURE IN ORDER FOR YOUR APPLICATION TO BE VALID).
- Your local dispatch office will fax your application to the Plan office.
- When you receive your claim stubs, fax or mail them into the Plan office as soon as possible.
You will be initially eligible for SUB benefits provided you have accumulated 26 credits in your account based on contributions made on your behalf by a contributing Employer within a 24 month period, and you meet the following requirements: - You are involuntarily separated from employment.
- You are signed on the out-of-work list at your Local 104 Dispatch Office.
- You are available for work.
- You are receiving State Unemployment Benefits, State Disability Benefits, Workers' Compensation Benefits, or are serving Jury Duty on a jury for 2 or more days.

| A. | If you wait and apply with EDD a few weeks after your last day worked, you will need to submit a letter of appeal when you submit your first payment stubs. Your appeal will be reviewed and approved or denied based on the circumstances. |

| A. | Unemployment - We must receive your claim stubs within 60 days of the date you are unemployed. Disability/Worker's Compensation - We must receive your claim stubs within 120 days of the date you become disabled. |

| A. | For those check stubs received by 5:00 PM on Wednesday, money will be deposited into your vacation account at the Local 104 Credit Union on Thursday by 12:00 PM. NO CHECKS ARE ISSUED BY THE TRUST FUND |

If you are faxing unemployment claim stubs,
please make sure your whole stub goes through. If your stub is cut off, we will
not be able to pay you. You can avoid the stub being cut off by tearing it off
and taping it in the middle of a blank piece of paper and then faxing it. |
